This policy defines the formal process for requesting, preparing, and releasing clinical reports at Relicus Psychological Services. It is designed to safeguard client confidentiality, ensure ethical practice, and comply with applicable professional guidelines and the Mental Healthcare Act, 2017.
Applicability
This policy is applicable to:
- All clients receiving psychological or counselling services through Relicus (both online and offline).
- All mental health professionals, clinical staff, administrative personnel, and coordinators involved in client documentation and information handling.
Policy Overview
Relicus upholds strict standards of privacy and ethical responsibility. Clinical reports are confidential documents and will be issued only to the concerned client after proper identity verification.
No clinical report will be shared with any external individual or organization without clear, written authorization from the client.
Procedure for Clinical Report Requests
4.1 Mode of Request
Clients must submit a formal request via email to:

4.2 Mandatory Details
Subject Line: Clinical Report Request – [Name of Therapist]
Email Content:
- • Client’s Full Name
- • Treating Therapist’s Name
- • Registered Contact Number and Email ID
- • Purpose of the Report (e.g., personal records, medical reference, academic, legal, or other)
- • Preferred Mode of Delivery (Email or WhatsApp, with exact contact details)
*Incomplete requests will be kept on hold until all information is provided.
Report Preparation Timeline
Within two working days after receipt of a complete and valid request.
- If the assigned therapist is unavailable due to leave or other commitments, the client will be informed of the revised timeline.
- Relicus does not accommodate urgent or same-day requests unless there is a documented medical necessity.
Confidentiality, Privacy & Verification
Reports will be released only through verified contact details registered with Relicus.
- Clinical reports will not be disclosed to employers, family members, partners, legal representatives, or any third party without written consent.
- Identity verification is mandatory prior to report release.
Ethical & Clinical Safeguards
Relicus reserves the right to:
- Refuse requests that may compromise client well-being, ethical standards, legal compliance, or clinical responsibility.
- Provide limited or summary reports when full disclosure may pose psychological or emotional risk to the client.
08. Professional Disclaimer
Clinical reports issued by Relicus are formal therapeutic documents prepared based on clinical sessions, assessments, and professional judgment.
- Are not character references, moral certificates, or validation documents.
- Are not intended to support personal conflicts, relationship disputes, workplace disagreements, or legal strategies.
Relicus does not alter, customize, or draft reports to influence legal proceedings, justify behaviour, or support adversarial purposes. Any request to manipulate report content will be declined.
Policy Compliance & Violations
Any instance of identity misrepresentation, unauthorized access, or misuse will result in:
- Immediate suspension of report processing.
- Possible disciplinary and legal action under applicable data protection and privacy laws (Digital Personal Data Protection Act, 2023).
